| NCSM Summer Camp |

To secure a spot in a summer class you must:
1. fill out this registration form (1 form for each child)
2. place a non-refundable deposit of $50.00/camp/student by May 1, 2007
(which will be applied towards the full tuition).
REGISTER BEFORE MAY 1, 2007 & GET $10.00 CREDIT OFF THE CAMP TUITION
Non NCSM Student must include $20.00 Registration fee/student
Late registrations will be taken on a first come, first serve basis.
Summer Camp Programs:
MusicArts Camp (for Pre-2nd grade & 3rd-5th grade)
NCSM Summer Chamber Workshop (for String, Woodwind, Guitar & Piano)
